How Pines Recovery Life uses and discloses your Protected Health Information under HIPAA and 42 CFR Part 2. Effective April 23, 2026.

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Our commitment

Pines Recovery Life is committed to protecting the confidentiality of your Protected Health Information (PHI). This Notice describes our legal duties and privacy practices under the Health Insurance Portability and Accountability Act (HIPAA) and 42 CFR Part 2, the federal regulation that provides additional protection for substance-use-disorder treatment records.

How we use and disclose your PHI

We may use and disclose your PHI in the following ways:

For treatment

We use PHI to provide, coordinate, and manage your treatment — including sharing information among our clinical team (physicians, therapists, nurses, case managers) and with other healthcare providers involved in your care, subject to applicable 42 CFR Part 2 consent requirements.

For payment

We use and disclose PHI to bill and collect payment for services — including verifying coverage with your insurance carrier, obtaining authorization, and submitting claims.

For healthcare operations

We use PHI for internal quality improvement, clinical training, accreditation reviews, and business operations — always in accordance with applicable privacy regulations.

Uses requiring your written authorization

Under 42 CFR Part 2, most disclosures of substance-use-disorder treatment records require your written, signed consent specifying the recipient and purpose. This includes disclosures to:

  • Family members, friends, employers, or attorneys;
  • Other treatment providers (except in specific emergencies);
  • Insurance carriers beyond what’s necessary for payment;
  • Any other third party.

You may revoke authorization in writing at any time, though revocation does not affect disclosures already made.

Uses without your authorization

Limited disclosures are permitted without your authorization, including:

  • To prevent a serious, imminent threat to health or safety;
  • To report suspected child abuse or neglect, as required by law;
  • For medical emergencies requiring immediate treatment;
  • In response to a valid court order specifically authorizing disclosure of 42 CFR Part 2-protected records;
  • For research conducted under strict protocols with ethics-board oversight;
  • To federal, state, or local authorities when required by law.

Your patient rights

Under HIPAA and related regulations, you have the right to:

  • Access and copy your medical records. We will respond within 30 days.
  • Request amendment of records you believe are inaccurate or incomplete.
  • Receive an accounting of certain disclosures we have made.
  • Request restrictions on how we use or disclose your PHI. We are not always required to agree but will honor reasonable requests.
  • Request confidential communications — for example, requesting that we contact you only at a specific phone number.
  • Receive a paper copy of this Notice even if you receive it electronically.
  • File a complaint without fear of retaliation (see below).

Breach notification

In the event of a breach of unsecured PHI, we will notify affected individuals within 60 days, as required by HIPAA’s Breach Notification Rule.

Changes to this Notice

We reserve the right to change this Notice. The revised Notice will be effective for all PHI we maintain. The current Notice is always posted on this website.
