Every Florida Blue plan, explained plainly.
Coverage depends on your plan type. Here’s what typically applies — and how it affects in-network vs out-of-network reimbursement for addiction treatment.
HMO plans keep care inside a closed local network and require a primary care referral. Pines is not in those networks, so HMO plans are not accepted here. If you also carry a PPO, POS, or EPO plan — or Tricare, Humana Tricare, or VA Community Care — that is the pathway, and we can check it for you at no cost.

PPO plans allow out-of-network treatment at reduced reimbursement rates. No PCP referral needed. Best flexibility for choosing addiction treatment facilities. Most Florida Blue PPO members pay deductible + coinsurance to reach their out-of-pocket maximum.
EPO plans combine PPO’s no-referral convenience with HMO’s in-network-only restriction. Out-of-network is not covered except emergencies. Our team confirms Pines is in your EPO network before admission.

Medicare and Medicare Advantage plans are not accepted at Pines. If you carry secondary commercial PPO coverage, or Tricare, Humana Tricare, or VA Community Care Network, we can verify that instead. Call us and we will tell you where you stand in one conversation, without any obligation.
Medical necessity, not red tape.
Florida Blue follows ASAM criteria for medical necessity. Coverage is authorized in stages as your clinical picture evolves — here’s what that typically looks like.
Medical Detox
Typically authorized for 3–7 days depending on substance, vitals, and withdrawal severity (CIWA-Ar for alcohol, COWS for opioids). Concurrent review extends coverage if withdrawal protracts. MAT medications (buprenorphine, methadone, naltrexone) are covered when clinically indicated.
Residential Inpatient
Typically authorized in 5–7 day increments, 14–30 days total for most patients with medical necessity. Co-occurring mental health treatment is included under parity. Case managers submit ASAM-compliant documentation to support continued stay when clinically indicated.
Aftercare Coordination
Your Florida Blue plan also covers step-down care through our partner network — outpatient, IOP, PHP, sober living referrals. Discharge planning confirms in-network continuity of care so your recovery doesn’t break at discharge.
Your out-of-pocket, ballpark.
Florida Blue members typically pay three things: the plan deductible (if not met), coinsurance on authorized services, and any facility fees beyond plan allowables. Your out-of-pocket maximum caps the total.
What most patients don’t realize: if you’ve already hit your deductible and out-of-pocket max for the year, treatment may cost you zero. Our financial counselors walk you through the exact numbers before you admit — nothing hidden.
