Access Standards Drive Time · Wait Time

If VA cannot provide care within 30 minutes’ drive time for mental health / substance use, or within 20 business days wait time for specialty care, the veteran may be eligible for community care.

Best Medical Interest Clinical Judgment

VA provider determination that community care is in the veteran’s best medical interest — for example, continuity of care with an existing private provider or specialized program.

VA Quality Standards Service Line Review

If the VA facility’s care for a specific service line does not meet quality standards, veterans may choose community providers that do.

Grandfathered 40-Mile Legacy Eligibility

Veterans who qualified under the former Veterans Choice 40-mile rule may remain eligible for community care under specific circumstances.

Authorized in 5–7 day increments with concurrent review. Co-occurring PTSD, TBI, depression, chronic pain — the conditions veterans frequently carry — treated alongside substance use under the same authorization.

Phase Three

Aftercare Coordination

Discharge planning coordinates with VA medical center, VA mental health services, and veteran-peer community resources. Continuity of care back to VA for ongoing support is prioritized.
