Empire offers a full commercial product line across downstate New York. Here’s how each plan type affects coverage for addiction treatment at Pines.

Empire EPO In-Network · No Referral

EPO plans combine PPO’s no-referral convenience with HMO’s in-network-only restriction. Out-of-state treatment processed via BlueCard; your admissions counselor confirms reimbursement before admission.

Empire PPO No Referral · OON Eligible

PPO plans allow out-of-network treatment at reduced reimbursement rates. No PCP referral needed. Most Empire PPO members pay deductible + coinsurance to reach their out-of-pocket maximum.

Empire HMO Not Accepted

HMO plans keep care inside a closed local network and require a primary care referral. Pines is not in those networks, so HMO plans are not accepted here. If you also carry a PPO, POS, or EPO plan — or Tricare, Humana Tricare, or VA Community Care — that is the pathway, and we can check it for you at no cost.

Empire Essential Plan NY Basic Health Program

A NY-state subsidized plan for moderate-income residents who don’t qualify for Medicaid. Essential plan covers substance use treatment under parity; out-of-state access is more restrictive than commercial plans.

Empire HealthPlus NY Medicaid Managed Care

Empire’s Medicaid plan. Out-of-state residential rehab is generally restricted except in specific medical-necessity exceptions. Our admissions team reviews your benefits carefully.

Empire MediBlue Medicare Advantage

Medicare Advantage plans follow specific rules for substance use treatment, including prior authorization and facility certification. Our admissions team navigates the Medicare pathway.

What Empire Covers at Pines

Empire applies ASAM criteria plus New York State’s Timothy’s Law parity provisions. Coverage is authorized in stages that reflect both federal and state standards.

Phase One

Medical Detox

Typically authorized 3–7 days depending on substance and withdrawal severity (CIWA-Ar for alcohol, COWS for opioids). Concurrent review extends coverage as needed. MAT covered when clinically indicated.

Phase Two

Residential Inpatient

Typically authorized in 5–7 day increments, 14–30 days for most patients with medical necessity. Co-occurring psychiatric conditions treated under the same authorization per MHPAEA and NY parity rules.

Phase Three

Aftercare Coordination

Discharge planning confirms NY-based step-down care — outpatient, IOP, PHP, sober living — through our partner network, matched to your Empire benefits and your home location.

Yes, on the EPO and PPO plans we accept. Empire covers medically necessary detox and residential inpatient under parity. Empire HMO, HealthPlus (Medicaid), and MediBlue (Medicare Advantage) are not accepted.
How is Empire different from Excellus BCBS?+
Empire covers downstate NY (NYC, Long Island, Westchester). Excellus BCBS covers upstate NY (Buffalo, Rochester, Syracuse, Central NY). Both are accepted at Pines. Check your member ID card for the logo.
Can I use my Empire plan in Florida?+
Yes. Empire members access the Pines Recovery Life campus in Pembroke Pines, Florida via BlueCard. Your Empire plan processes the claim at your plan’s in-network or out-of-network rates. Our admissions team handles BlueCard coordination directly.
What is Timothy’s Law?+
New York’s 2006 parity law requiring commercial insurers to cover at least 20 outpatient visits and 30 inpatient days for mental health and substance use. Empire must meet or exceed these minimums; additional coverage authorized per ASAM criteria.
Does Empire cover MAT?+
Yes. Empire commercial plans cover medication-assisted treatment — buprenorphine, methadone, naltrexone — when clinically indicated. Coverage falls under the same parity rules as other BCBS plans.
What will I pay out of pocket?+
