Medical Detox

Authorization depends on the substance and how severe withdrawal is (CIWA-Ar for alcohol, COWS for opioids). Concurrent review extends coverage if withdrawal protracts. MAT is covered when clinically indicated.

Phase Two

Residential Inpatient

Authorized in increments and extended for as long as medical necessity holds. Co-occurring psychiatric conditions are treated under the same authorization per Mental Health Parity.

Aftercare coordination

Discharge planning confirms in-network step-down care (outpatient, IOP, PHP, sober living) through our partner network, matched to your UHC benefits and home location.
