Coverage depends on your plan type. Here’s what typically applies — and how it affects in-network vs out-of-network reimbursement for addiction treatment.
HMO plans keep care inside a closed local network and require a primary care referral. Pines is not in those networks, so HMO plans are not accepted here. If you also carry a PPO, POS, or EPO plan — or Tricare, Humana Tricare, or VA Community Care — that is the pathway, and we can check it for you at no cost.
A broader HMO offering wider provider access and more employer group adoption. Coverage for detox and residential is guaranteed under MA parity and MHPAEA; tiered cost-sharing may apply.
PPO plans allow out-of-network treatment at reduced reimbursement rates. No PCP referral needed. Best flexibility for choosing addiction treatment facilities outside of MA.
A point-of-service plan blending HMO and PPO. In-network cost-sharing is lowest; out-of-network available with higher coinsurance. Mental-health-parity protected.

BCBS MA combines federal MHPAEA with Massachusetts-specific parity rules. Coverage is authorized in stages that reflect both ASAM criteria and state minimums.
Medical Detox
MA guarantees at least 14 days of inpatient detox coverage without utilization review when clinically indicated. CIWA-Ar monitoring for alcohol, COWS for opioids. MAT medications covered when indicated.
Residential Inpatient
MA guarantees at least 14 days of post-detox residential without additional utilization review. Beyond that, 5–7 day increments per ASAM criteria. Co-occurring conditions treated under same authorization.

Discharge planning confirms MA-based step-down care — outpatient, IOP, PHP, sober living — through our partner network, matched to your BCBS MA benefits and your home location.
