HMO Blue Not Accepted

HMO plans keep care inside a closed local network and require a primary care referral. Pines is not in those networks, so HMO plans are not accepted here. If you also carry a PPO, POS, or EPO plan — or Tricare, Humana Tricare, or VA Community Care — that is the pathway, and we can check it for you at no cost.

Access Blue Broader Network HMO

A broader HMO offering wider provider access and more employer group adoption. Coverage for detox and residential is guaranteed under MA parity and MHPAEA; tiered cost-sharing may apply.

BCBS MA PPO No Referral · OON Eligible

PPO plans allow out-of-network treatment at reduced reimbursement rates. No PCP referral needed. Best flexibility for choosing addiction treatment facilities outside of MA.

Blue Choice Tiered Network

A point-of-service plan blending HMO and PPO. In-network cost-sharing is lowest; out-of-network available with higher coinsurance. Mental-health-parity protected.

Blue Cross MassHealth Not Accepted

Medicaid and Medicaid managed care plans are not accepted at Pines, and they generally do not extend to out-of-state residential treatment. If you have other coverage, or Tricare, Humana Tricare, or VA Community Care, we can verify that. If not, our team can talk through what other options exist.

BCBS MA Medex Medicare Supplement

Supplemental plans that pair with Original Medicare. Substance use treatment follows Medicare’s coverage rules with supplemental coinsurance and deductible reduction.

MA parity meets ASAM, for your stay.

BCBS MA combines federal MHPAEA with Massachusetts-specific parity rules. Coverage is authorized in stages that reflect both ASAM criteria and state minimums.

Phase One

Medical Detox

MA guarantees at least 14 days of inpatient detox coverage without utilization review when clinically indicated. CIWA-Ar monitoring for alcohol, COWS for opioids. MAT medications covered when indicated.

Phase Two

Residential Inpatient

MA guarantees at least 14 days of post-detox residential without additional utilization review. Beyond that, 5–7 day increments per ASAM criteria. Co-occurring conditions treated under same authorization.

Phase Three

Aftercare Coordination

Discharge planning confirms MA-based step-down care — outpatient, IOP, PHP, sober living — through our partner network, matched to your BCBS MA benefits and your home location.

Yes, on the PPO and POS plans we accept. Blue Cross Blue Shield of Massachusetts covers medically necessary detox and residential treatment under parity. HMO Blue and Blue Cross MassHealth (Medicaid) are not accepted.
How does MA state parity protect me?+
MA Chapter 258 and the Access to Treatment Act supplement federal MHPAEA rules. BCBS MA must cover at least 14 days of inpatient detox and 14 days of post-detox residential without requiring additional utilization review when clinically indicated. Additional days follow ASAM criteria with standard concurrent review.
Can I use my BCBS MA plan in Florida?+
Yes. BCBS MA members access the Pines Recovery Life campus in Pembroke Pines, Florida via BlueCard — the national BCBS reciprocity program. Your MA plan processes the claim at your plan’s in-network or out-of-network rates. Our admissions team handles the BlueCard coordination directly.
HMO Blue vs Access Blue vs PPO — which is best for rehab?+
For treatment at Pines, a PPO or POS plan is what works, because it allows out-of-state and out-of-network care. HMO Blue is not accepted, since HMO plans restrict care to a closed in-state network. If you are unsure which you hold, we will read your card with you.
Does BCBS MA cover medication-assisted treatment?+
Yes. BCBS MA covers MAT — buprenorphine, methadone, naltrexone — when clinically indicated. Coverage falls under the same parity rules as other medical care with no additional prior authorization hurdles beyond standard clinical review.
What will I pay out of pocket?+
Depends on deductible, coinsurance, and OOP max. MA parity prohibits higher cost-sharing on substance-use care than medical/surgical. Many patients who have met their deductible pay little to nothing.
