The three levers that determine your actual cost.
Every commercial plan pays for rehab using the same three financial levers. Understanding them lets you walk into admission with clarity, not surprises.
Deductible
The annual amount you pay before insurance starts covering a percentage. Plans range from $0 to $8,000+. Many patients mid-year have already met their deductible on other medical care — that can meaningfully reduce treatment cost.
Coinsurance
Your percentage share after the deductible is met. Typically 10–30% of the plan’s allowed amount for in-network care. Under Mental Health Parity, addiction treatment must be at the same rate as medical/surgical care.
Out-of-Pocket Maximum
Once you hit your annual OOP max, your insurance pays 100% of covered in-network services for the rest of the year. ACA-compliant plans cap OOP at $9,450 individual / $18,900 family (2025 limits).
